Metal tab at top is designed to be used to secure adapter to outlet using center plate mounting screw.  Therefore, the blades are not polarized so the adepter can be used and secured both on the top or the bottom outlet.  This feature is desired because often a ground wire is attached internally to a 2 prong outlet and the center screw passes the ground to the adapter and then to the 3prong grounding post of your device cord.
