La infeccion quirurgica es la complicacion mas frecuente del procedimiento quirurgico. Su importancia es tal, que los indicadores de infeccion hospitalaria se aceptan como uno de los parametros de calidad asistencial mas importantes que genera un centro sanitario. No existen en la literatura medica estudios con suficiente rigor estadistico que establezcan el riesgo relativo y el beneficio de la administracion de antibioticos profilacticos en el postoperatorio o por periodos prolongados. Una profilaxis incorrecta se acompana de consecuencias no deseadas del uso del antibiotico como la aparicion de efectos secundarios, cepas bacterianas resistentes, infecciones secundarias y aumento del gasto. Como regla general ampliamente aceptada, la profilaxis debe suspenderse siempre tras un maximo de 24 horas despues del procedimiento quirurgico, y deseablemente deberia realizarse una dosis unica repetible si la cirugia se prolonga o hay perdidas sanguineas importantes. Sin embargo no disponemos de verdaderos estudios prospectivos que analicen con suficiente profundidad la eficacia y seguridad de una u otra pauta.
