Synopsis
The EPA investigation of a 1994 chemical plant tragedy concluded that "the explosion resulted from a lack of written safe operating procedures..." While good written procedures can't guarantee zero accidents, they can reduce the number of accidents caused by human error. This new book shows how to remedy this problem through selecting and implementing actions that promote safe, efficient operations and maintenance, improve quality, continuity, profitability and cost control, build upon and record process experience, and promote the concept that operating and maintenance procedures are vital plant components. It includes practical samples of procedure formats, checklists and many references., The EPA investigation of a 1994 chemical plant tragedy concluded that "the explosion resulted from a lack of written safe operating procedures " While good written procedures cant guarantee zero accidents, they can reduce the number of accidents caused by human error.

The book is fantastic, just as described by the seller. Packaging was very good, it was shipped in a priority mail box. Seller was going to ship without tracking, but I needed it to have a tracking number, seller accomodated with no problem. I would absolutely buy again from them!

Packaging leaves much to be desired. This album was packaged in a flattened USPS unfolded box, then wrapped in what felt like heavy duty trash bag material. I’m really disappointed in the condition item arrived; a fragile marking could’ve helped, but wasn’t even marked. The described item arrived, ??sealed??, but damaged. Transaction process was smooth, but kinda wish it didn’t. I would have reservations dealing with this seller. Again I wasn’t expecting pristine condition, but come-on.

Hello. We are sorry to hear about your feedback. The creasing in on the sleeve of the record is actually in the photos of our listing. This is not damage from shipping or packaging. The creasing was already on the record sleeve, its just under the plastic. Again, we are sorry about your experience and do hope that you give us another chance one day. Have a lovely rest of your day.
